In November 2025, the FDA announced that it is initiating the removal of the broad “black box” warnings from menopausal HRT products that treat symptoms such as hot flashes, night sweats, and vaginal dryness.
These warnings had long communicated major risks like cardiovascular disease, breast cancer, and dementia — warnings placed after the landmark 2002 Women’s Health Initiative (WHI) trial.
The updated labeling is based on a comprehensive review of current scientific evidence, reflecting a more nuanced understanding that the benefit-risk profile of HRT differs depending on timing (relative to menopause onset), formulation (systemic vs topical), and individual health status.
